Working with people in the hospital setting 



By 2025 the Government is aiming for a 50% reduction in the number of people under 65 being admitted to aged care.

To meet this target, support coordinators will assist people with complex needs in hospital to get and implement an NDIS plan that can support discharge back into the community.


We are holding this workshop to build the capability of support coordinators to work effectively with people in the hospital setting. This will help ensure that people can discharge with the appropriate health, NDIS and mainstream and informal supports in place.



Support coordinators will have an understanding of the key drivers of hospitals and the NDIS and how to work collaboratively across the interface.

The workshop will be presented by Penny Paul, senior policy and research officer at the Summer Foundation. Penny also leads UpSkill, an initiative to build the capability of support coordinators.

Health staff from Austin Health will also be key contributors to this workshop.
