Working with people in hospital - Live Webinar

The Government has set new targets around young people in aged care:

  • No people under 65 entering residential aged care (RAC) by 2022
  • No people aged under 65 living in RAC by 2025

Providing support coordination to people in hospital is a key to reducing the number of younger people being admitted to RAC. To meet the targets, support coordinators need to assist people with disability to leave hospital well with an appropriate mix of NDIS and mainstream supports.
 

TOPICS INCLUDE:

  • Key drivers for hospitals
  • NDIS and health working together
  • Early interim NDIS plans for timely discharge
  • Housing supports available under the NDIS including the eligibility requirements for Specialist Disability Accommodation (SDA)
     

OUTCOMES:

Support coordinators will have an understanding of the key drivers of hospitals and the NDIS, and how to work collaboratively across the interface. They will be able to work with people in the hospital setting to prepare for discharge with appropriate health, NDIS and mainstream supports in place. This live webinar will include learning checks, reflective practice and Q&A.
 

PRESENTERS:

Penny Paul is our Senior Policy and Research Officer. She is rolling out the Summer Foundation UpSkill initiative to build the capacity of NDIS support coordinators and allied health professionals. 

 Michelle Bult is our Health Engagement Lead, working with hospitals to streamline discharge practices and processes in the context of the NDIS.
