Summer Football - TEAM Registration
IMPORTANT: PLEASE READ ALL NOTES BELOW BEFORE PROCEEDING
Please complete this brief registration form if you have formed a team for the Forest Summer Football Competition 2021/22. Only one form per team (not per player) needs to be completed. 4-a-side matches need a minimum of 4 players to register a team, and 6-a-side matches need a minimum of 6 players per team. We recommend that each team has at least 2 additional players for substitutions (and to cater for illness/injury). Team Managers must be at least 18 years of age.
NOTE: This form is not to be completed for individual players.
PLEASE CHECK THAT YOU HAVE ALL THIS INFORMATION IN ORDER TO COMPLETE TEAM REGISTRATION
- A name for your team
- Names (first name and surname) of all players
- Shirt sizes for all players (please refer to the shirt sizes below in order to choose the most appropriate size for your player)
- Shorts and socks sizes (If required, payment not needed until collection)
- Winter Soccer Division/Player Level OR Social Player Level
Shirt sizes are available in:
YOUTH - 4, 6, 8, 10, 12
ADULT - XS, S, M, L, XL, XXL
There is also an option to indicate if a player already has a shirt from the 2018/19/20 competition. Shirts from any other competitions prior to 2018 cannot be worn.
PLEASE NOTE: all players in a team must wear the same shirt colour.

Completion of Registration and Team Approval
After this form is submitted, please wait for an email to confirm when your team players can register individually in PlayFootball with the Team Name nominated in this form. Player registration is due to open in September.
The information collected in this form will be matched against each individual player's registration. Submission of this form DOES NOT GUARANTEE the approval of your team into the Forest Summer Soccer competition. Once all players have registered individually, team approval will only occur as long as there is availability in the competition.
PLEASE NOTE: If an Age Group reaches full capacity, you may still register your team to be placed on a WAITING LIST only. You will be informed immediately if your team is on a waiting list.
Changes to your team (for example additional players, removal of players or change/addition of manager) are still allowed prior to the start of the competition. If there are any changes the Team Manager must send an email to firstname.lastname@example.org to ensure we can match up information collected in TEAM registration with individual PLAYER registration. Team approval will be delayed if team changes are not advised.
