Council collects the personal information on this form to administer your and your child’s participation in parent groups run by Council.

Privacy

As you may appreciate, Council is affected by social distancing restrictions and parent group activities are currently run on Zoom. You will be sent a password-protected Zoom link to participate in the session, and if you participate in the session, your name and video image will be available to others in the group.

Please note that the link is intended for registered parent group participants only and should not be forwarded. If you would like to invite someone else to the group, please contact maternal and child health administration on 9524 3403 and we will provide them with the link separately.

Further, to ensure that everyone feels respected and comfortable participating in the group, video recording for parent group sessions will only be enabled with host permission. We ask that you refrain from recording and sharing any of the group’s activities unless you have received permission. Council accepts no liability for any loss or damage caused by unauthorised use or disclosure of information contrary to the expectations outlined in this paragraph, or misuse of the Zoom service or platform.
