I acknowledge that due the current Covid-19 restrictions ,as a parent /carer, I am not to enter the school premises.

I understand that Waverley College Swim School and its staff shall be released from and shall not incur any responsibility or liability whatsoever for any accident or for any damage to loss of property of the applicant. I hereby authorise Waverley College Swim School staff to organise medical treatment as they see necessary at my expense.
