Serious Illness Conversation Guide Workshop 2024
Introducing Shared Goals of Care (SGOC) and Serious Illness Conversation Guide training (SICG).
The purpose of this training is to provide a framework where discussions establish the basis for Shared Goals of Care decisions. This supports provision of high quality, goal concordant care.
When shared goals provide the basis for clinical treatment plans, there is less risk of a patient receiving unwanted or unwarranted treatments if their condition deteriorates.
The workshop will support a timely serious illness conversation:
• a patient’s understanding of their illness
• their preferences for information
• sharing a prognosis.
• exploring personal goals, fears and worries, abilities most important to their daily
life and trade-offs
• discussing a plan and way forward.
• Documenting the discussion on E-SGOC form (Replaces Blue Resus Form )
