About
Join us in exploring healthcare, social care, acute and community services to deliver person-centred healthcare transition across Lancashire and South Cumbria.
Join leaders and practitioners from across the system—including NHS England, adult social care, youth services and mental health pathways, alongside a specialist workshop from the Kentown Palliative Care Team.
Hosted by the Health Innovation Campus, Lancaster University, this Community of Practice brings together system leaders, clinicians and partners to share learning, align priorities and drive improvement in transitions across physical and mental health preparing our young people for adulthood.
The day will consist of two sessions.
The morning session will be opened by Jon Soros, Principal Social Worker Adults, Health and Wellbeing - Lancashire County Council and feature talks from leading experts including, Vimal Bedia, Policy Manager - Children and Young People's Transformation Programme, NHS England, Kelvin Sarsfield, Children and Young People's Place Lead All-age Mental Health Team - NHS Lancs & Sth Cumbria Integrated Care Board and Natalie Stornelli, Social Worker Service Manager - Lancashire County Council.
The afternoon session will be an interactive workshop led by the Kentown Palliative Care Nursing Team to explore good practise around young adults with life limiting conditions and the care, support and preparation themselves and their families need for leaving children services.
