HACC-PYP Assessment & Planning
This interactive workshop will support staff to build on their assessment and planning skills and consider ways to conduct more meaningful, person centred assessments with HACC PYP clients, and create care plans that add value for staff, clients and carers.
Facilitated by Kate Pascale & Associates, the workshop will be for New Staff or staff wanting more comprehensive training. The training will comprise of 1 full day face-to-face on the 17th September in Ararat, followed by a 3-hour virtual follow-up session on the 8th October via Zoom.
