Join us to explore how CareChoice partners closely with hospital teams to conduct comprehensive assessments of each client’s clinical needs, risks, funding, and support requirements—ensuring a safe, appropriate, and sustainable discharge home.Learn how individualised care plans, tailored staffing models, and our bespoke Client High Intensity Training (CHIT) program equip qualified staff with the skills and knowledge needed to safely support complex, client-specific needs.
We’ll also walk through what happens post-discharge, including how our team supports a smooth transition toward improved quality of life through goal setting, community engagement, and the delivery of meaningful, person-centred supports.
Led by our Registered Nurse clinical team, this session will highlight how ongoing data collection, regular staff engagement, and collaboration with support coordinators and allied health professionals ensure each program remains responsive, evidence-based, and aligned with the client’s evolving needs.
