This masterclass takes managers and clinical leads past the definitions and into the judgment calls, and shows you how to put a defensible written rationale behind every call you make.

1. The eight types, worked through the grey. Every reportable incident type under section 16 of the Aged Care Act 2024, including the newly redefined neglect and the combined unexplained absence and missing person category, with the borderline calls that most often go wrong: cognitive impairment and consent, cumulative minor incidents, chemical restraint versus treatment, and the absence that ended well.

2. Priority 1 vs Priority 2, in practice. The current Priority 1 definition and its practitioner-treatment test, the triage workflow, the 24-hour and 30-day clocks and when they really start, classifying up under uncertainty, and who owns the decision, including on a Saturday night.

3. The defensible decision trail. What survives an audit two years later: contemporaneous records, reasoning against the statutory definition, and equal rigour for decisions NOT to report.

4. Learning from serious incidents. A complete worked root cause analysis of a serious incident: five whys, contributing factors, strong versus weak controls, open disclosure and governing body reporting under Outcome 2.5.
