The aim of advance care planning (aCP) is to align a person’s wishes and preferences with the care they receive if a time comes that they cannot make or communicate decisions. Contemporary understandings of advance care planning recognise that it can take many forms and it is a process that respects a person’s readiness to engage in conversations and/or recording of wishes.
