Re-credential as a Shared Maternity Care Affiliate (SMCA) General Practitioner
1 January 2020 – 31 December 2022

Thank you for your association with Mercy Hospitals Victoria Ltd, The Royal Women’s Hospital, Western Health and Northern Health in providing shared maternity care.
We invite you to apply to re-credential as a Shared Maternity Care Affiliate for the triennium 1 January 2020 – 31 December 2022.

Step 1: Register to re-credential online via TryBooking. This enables us to capture your contact information in order to start the application and email you the requirements to complete the application.

- Complete your details

- Refer to the step-by-step guide attached to your invitation email 

Step 2: You will receive a transaction email (this email confirms you have commenced registration to re-credential)

Step 3: You will receive a confirmation email including the documentation required to complete your application

Step 4: Re-credentialing documents are due by 1st November: including the signed Shared Maternity Care agreement (this was attached to your invitation email)

Applicants will be advised of the outcome of their application by the 31st January 2020.
