When Chronic Disease and Mental Illness Coexist

Join industry and Silverchain health care experts to learn more about chronic and complex diseases towards achieving effective integrated care and improving patient outcomes.

Silverchain is hosting a series of free interactive education sessions to connect doctors, GP registrars, practice managers, nurse practitioners, and specialists with practical information and referral pathways across various topics. This event provides you with 1.5 CPD self-logged hours (one hour educational activity, 0.5 hours for reviewing performance) and includes a two-course dinner and refreshments.

Industry panellists

Three industry experts will share their knowledge on the night and take part in panel discussions following their presentations: 

Dr Stefan Schutte (Psychiatrist, Head of Department – Psychiatry, St John of God Midland Public and Private Hospitals)

• A qualified perinatal and consultation-liaison psychiatrist with experience in effectively treating patients with a wide range of conditions, Dr Schutte applies a well-rounded approach to treat the whole person through medications and psychotherapy.
• Topic: Understanding the impact of mental illness on chronic disease in goal setting, motivation, and successful self-care. 
• Learning outcome: Greater understanding of why patients with chronic disease are at risk of poor self-care from a neurobiology and psychiatry point of view and guiding principles for helping them achieve their health goals.

Dr Michael Christmass (Consultant Addiction Medicine – Fiona Stanley Hospital and Next Step Drug and Alcohol Services)

• As a Fellow of the Australasian Chapter of Addiction Medicine (Royal Australasian College of Physicians), Dr Christmass is a consultant at Addiction Medicine for the community substance use disorder treatment service. He contributed to establishing the first stimulant use disorder outpatient treatment clinic in Western Australia.
• Topic: Determining the impact of substance use on chronic disease and evidence-based community management of alcohol and other drug rehabilitation.  
• Learning outcome: Ability to determine the appropriate management plans and referral pathways for patients seeking to withdraw from alcohol or other drugs with community support.

Jaclyn Geraghty (Program Manager Primary Care and Chronic Disease - Silverchain)

• With 10 years’ experience in chronic disease self-management and care coordination using the Flinders Chronic Condition self-management program, Ms Geraghty has embedded the Flinders model into Silverchain’s practice and supports chronic conditions teams to deliver care to complex and vulnerable clients.
• Topic: A case-based discussion – using the Flinders models of care in supporting patients with chronic disease.  
• Learning outcome: Understanding the key features of the Flinders model to identify patients who might benefit from its application and suitability for community supported program
