Welcome to the registration form for the Sacrament of Confirmation.Please fill out 1 registration form per child receiving the Sacrament.
Child must be in year 6 or above at school, and be Baptised and have completed the Sacrament of Reconciliation and First Holy Communion, to be eligible for Confirmation.
Criteria for choosing sponsors are: he/she must be over the age of 16 and be a Confirmed Catholic themselves.
Proof of the sponsor's Confirmation is required. This can be obtained by calling the Church where they were Baptised or the Church where they were Confirmed if different from place of Baptism.
