Best Practice Forum - Moving Forward
You are invited to the annual forum for CHSP service providers, bringing together Program Managers, Allied Health Clinicians, Nurses, Regional Assessment Services staff and other Health Professionals from across rural Victoria – Grampians and Barwon South West regions.
This one-day forum is an initiative of the Grampians Region Sector Development Team, which aims to build collaborative partnerships within the CHSP sector to support change management, providing a focus on the aged care reforms, the sharing of good practice, and encouraging inclusive access and person-centred care.
This year’s Best Practice Forum ‘Moving Forward’ will focus on the implementation of activities and plans to ensure compliance and quality service provision moving towards the Support at Home Program. Everything from Quality Standards to lived experiences will shape a day of information sharing and learning.
