This workshop is part of the regional approach to strengthening ACP through the Greater Choice for At Home Palliative Care project by Western Victoria Primary Health Network.Topics will include:
• Understand the ACP process
• Have the conversation: Values, goals & preferences
• Assist a person to appoint a Substitute Decision Maker
• Assist a person to document their preferences
• Understand the law, process & systems that support ACP
