Modern societies face a growing challenge to support older people in their own homes until they die. Unfortunately, cognitive and physical frailties increasingly lead to aged care placements. These residents have high care needs and 90% will die in the facility. Hence, the delivery and importance of palliative care is integral to the support for older people, their families and those that care for them. Access to palliative care has been patchy and fragmented and in need of urgent review.
