Join CareChoice for a practical and informative webinar exploring how to safely and effectively support justice-involved participants with disability as they transition from custodial or forensic settings into the community.

Participants involved with the justice system often face significant barriers when accessing appropriate housing and supports - including fragmented planning, complex risk considerations and unclear system responsibilities. This session brings together disability, legal, compliance and clinical perspectives to examine what works in practice when supporting safe, structured and person-centred transitions into community-based SDA.

Designed for clinicians, support coordinators, justice workers and other referrers working in complex forensic contexts, this webinar aims to provide clarity, confidence and practical next steps.
