MDT care in Residential Aged Care Workshop - North Lakes

Tuesday 2 June 2026 | North LakesBrisbane North PHN are hosting an evening workshop on MDT care to explore the importance of, and ways to improve, MDT working in RACHs. This initiative has come from feedback recognizing that MDT working is more complex in the RACH setting, where members of the team are coming together from various organizations to collaborate on providing the best possible care for residents. This is a unique opportunity to come together and explore ways to improve MDT working in your facility, and how this can be supported through the General Practice in Aged Care Incentive (GPACI) and the Aged Care On-site Pharmacy measure (ACOP).
The evening will be split into two parts, opening with a presentation over dinner by Tracey Johnson (Chief Executive Officer) and Dr. Suzanne Williams (Clinical Director) from Inala Primary Care (Sandstone Healthcare Group), sharing their expertise and experiences in improving Multidisciplinary Team (MDT) ways of working in Residential Aged Care Homes (RACHs) and care of older people in general practice. This will be followed by an interactive scoping and planning session, where participants who work together will have the opportunity to apply these learnings to their own specific contexts.
The objectives of the workshop are to help participants:
• understand their current challenges in delivering MDT ways of working
• scope opportunities to improve MDT ways of working in their own settings
• collaboratively identify specific models and solutions that would work for all parties providing MDT care
These workshops are free, with a two-course dinner and drinks included. There will be free parking on site.
Attendees will be provided with a report recently co-authored by Tracey which outlines the change strategy recommended to government to transition practices to more comprehensively use teams in care.
To get the most out of this event, we strongly recommend coordinating with a few members of your MDT to attend as a group, for example, a representative from the RACH, a GP that visits the facility, and an ACOP or Allied Health clinician who supports resident care. This will allow for conversations and planning in the interactive part of the evening to support practical outcomes.
