MONDAY 13TH APRIL- NATIONAL GALLERY OF VICTORIA AND FITZROY GARDENS EXCURSIONOur WhiSKHA Days School Holiday program is aimed at children aged 5-18 years with a mild-moderate disability. Participants are able to enjoy and experience a variety of activities, develop skills, interact socially, achieve goals and meet personal challenges in a safe and stimulating environment.
Our program schedule of excursions has been provided as a guide, but will remain flexible according to the needs, abilities and interest of the participants attending on the day.
Enrolment Process:
1. Register with Y Inclusion Services. If you are a new participant with us and don't have a current service agreement in place, please contact us first at info@yinclusion.org.au.
2. Complete this Application form and pay your out of pocket cost (only on selected days). Please note that refunds will only be made if a spot is not available or a programme is cancelled.
3. Holiday Program Coordinator will send a Service Agreement listing all NDIS fees and Confirmation of successful enrolment to the program (your place on the holiday program is pending a signed Service Agreement to be returned).
4. Reminder of upcoming booking will be sent 2 weeks prior to the program.
