Shared Care Planning Workshop
This online workshop focuses on the key elements of a best practice approach to shared care planning. This includes how agencies can develop and embed organisational systems and processes to support shared care planning across multiple services.
Information covered will include:
* The quality standards that relate to Goal Directed Care Planning (GDCP)
* How and when shared care planning adds value for clients and staff
* The key elements of shared care planning, Policies and Procedures
* Effective shared care planning tools and templates
* Key considerations for good practice
Target Audience:
CHSP staff responsible for the development and implementation of systems (care planning) to support service delivery (when multiple services are delivered) including quality improvement staff, program managers and team leaders.
