Creating effective shared care planning systems and tools
This 3 hour in person workshop is designed to support participants to understand how they can embed the principles of effective shared care planning.
Participants will be guided through a series of key questions that focus on decision points to create flexible, efficient policies that align with their own organisational systems and processes.
Participants will have the opportunity to review their existing systems and identify areas for improvement.
Participants registering to attend this session are required to have attended session 1.
