Residential aged care continues to face increasing clinical complexity, workforce pressures and system challenges. Stronger coordination between general practice, residential aged care and health system partners is essential to support quality care for residents.
EMPHN is hosting a series of Aged Care Collaborative Sessions across Eastern Melbourne. These sessions are designed to provide a practical forum for clinicians, aged care providers and system partners to come together, share local experience, and work through common challenges.
Sessions will focus on what is working in practice, where gaps or barriers exist, and what practical actions can be taken to improve communication, coordination and care outcomes. Participants will also hear perspectives from across the health system, including the Commonwealth, local health networks, Ambulance Victoria, the Victorian Virtual ED and Residential In‑Reach teams, to support shared understanding and alignment.
RACHs and GPs are encouraged to attend together to support collaborative discussion and shared solutions.
Session format:
In person event.Sessions will be delivered across three regions.
When registering, please select one session/location based on where you deliver care to residents most frequently to support meaningful, place‑based discussion relevant to their local context.
