Class of 2027 Year 12 Jersey

Please complete all aspects of the form to secure your Year 12 keepsake for 2027. A reminder- students are only permitted to wear the Year 12 Jersey to school on a Friday.

Please note that names can be your first name OR last name OR initials e.g. Briohny OR Hickey OR B.H.

Please be caps specific eg HICKEY or Hickey

You cannot have a nickname; shortened versions of your full name are considered eg Sam for Samantha. This is non-negotiable.

Leadership positions can be added when this process is finalised.
