Please complete this booking form to have your child/ren's photos taken during photo week Monday 25th - Friday 29th May. We will be taking photos each morning from approx 8.30am till lunchtime. We aim to take a mix of candid and posed photos and sibling photos will be taken where applicable.

There is a $25 booking per family, which is redeemable against your order.
