Youth Activity/Event Consent FormThank you for your interest in our youth groups. At the moment, all groups are full. Any new registrations will be placed on a waitlist and we will contact you if a spot becomes available.
To participate in any events, activities, or groups facilitated by ELAM, this form must be fully completed and signed by a parent or guardian for youth under the age of 16. By signing this form, you acknowledge and agree to the accuracy of the information provided and to the terms outlined by ELAM Inc. Please note: All youth activity events and group sessions must be booked exclusively through the ELAM website: \
https://www.elammoranbah.com/events. In the event that a youth participant is unable to attend a scheduled event, it is their responsibility to update their registration details promptly, thereby allowing another youth to participate in the open spot.
