We are trained to trust what we can see, touch and measure in front of a patient. Bone offers none of this. It sits in a clinical blind spot, so we read its decline as a soft-tissue problem and treat it as one.But the sequence is predictable. The orbit goes first, typically through the 30s and 40s, presenting as lower lid laxity, a hollow tear trough and lateral brow descent. The maxilla follows in the 40s and 50s, taking the bony platform out from under the midface fat and deepening the nasolabial fold. The mandible follows later, softening the jawline.
Resorption begins years before any of it becomes visible. Treating ahead of what you can see is a discipline regenerative work already demands of us. Bone asks for the same reasoning and gets it far less often, yet it determines where every regenerated layer above it will sit.
Structural replacement is not the same as volumising. A firm, high G’ hyaluronic acid filler provides projection and support at foundational anchor points and does a different job to anything else in your treatment toolbox.
