Join Wendy O’Meara for a practical session exploring Chronic Condition Management (CCM) - Understanding CCM Framework in Practice, Identifying team roles in practice for CCM; and application of practical strategies to strengthen CCM workflow.
This event is for practice managers, practice nurses, GPs and the broader primary care team.
About Wendy

Wendy currently works as a Practice Manager in a high-performing general practice in Gisborne, Victoria, with a team of 24 staff.
Over the past 16 years she has built this clinic into a large multi-disciplinary practice with a full range of allied health professionals and specialists attending regularly. Her practice achieved the AGPAL Rural Practice of the year in 2019.
She is passionate about general practice with a special interest in Medicare Benefits Scheme (MBS), chronic disease management, accreditation and mentoring of new doctors, registrars and medical students.
Wendy has worked as an education consultant since 2017, guiding GPs, practice nurses and practice managers in practices across Australia to achieve strong outcomes.
Learning outcomes

Understand the current Chronic Condition Management framework and MBS opportunities including identifying the key Medicare requirements, eligibility criteria, billing opportunities and compliance considerations relevant to general practice.

Identify the role of the Practice Manager and practice team in CCM, including establishing effective systems for patient identification and MyMedicare registration, recalls, care coordination, documentation, claiming and monitoring outcomes.

Apply practical strategies to strengthen CCM workflows within the practice, including optimising practice software, improving team-based care, reducing missed billing and patient care opportunities, supporting appropriate MyMedicare registrations and supporting sustainable delivery of chronic condition management services.

- Attendance and catering is free, but registration is essential.
- Places are limited, so register early to secure your place. Registrations close STRICTLY on Saturday October 31st at 5:00pm.
- Your registration will be taken as your RSVP that you will be attending.
