Integrated Care: Taking the First Steps

As the sector has demonstrated its resilience and flexibility during these exceptional times, it is time to reconnect, learn and reflect on the way forward, so we are excited to announce that our May Forum will be held face-to-face!
The recent Royal Commission’s Final Report emphasised several issues, both at the system and providers’ levels, and offered several recommendations.
Our May Forum will draw from a range of aged care experts, including allied health providers and GPs, on how we, as CHSP and HCP package providers, can work towards a more integrated approach, that encompasses the individual physical, mental and social needs of older people to remain at home longer.
Come along to learn, be inspired, reconnect to your colleagues, but more importantly, receive practical tips, tools and strategies that you can take back to your organisation so that you are equipped to provide a more integrated model of care within the community and/or an individual’s home.
Who to attend to Regional Forum?
This workshop is for CEOs, Board members, Managers, Coordinators, Team Leaders, Front-line Workers, Case Managers & Support Facilitators who are interested in implementing an Integrated Care service model within their organisation to empower both staff and service users.

Cost: $45 for Your Side Members; $60 for CHSP-funded service providers; and $75 others (this will contribute to the costs of lunch and morning tea)

Cancellation Policy
Payment is required to secure any booking. Cancellations received up to 7 working days before event is refundable. After that, cancellations are subject to the entire event fee. Please note that if you don’t cancel and don’t attend, you are still responsible for payment.
