This document affects your legal rights and obligations and those of the Minor. Please read it carefully. Do not sign unless you understand.

ACKNOWLEDGEMENT
By ticking the box below, I acknowledge that:
a) I am the parent/guardian of the Minor.
b) The Minor receives golf coaching by an 'expert instructor'.
c) The Activity involves inherent risks, including physical harm and property damage.
d) Others, including participants and bystanders, may cause harm or damage during the Activity.
e) Natural conditions in which the Activity occurs may vary without warning.
RISK WARNING
I acknowledge the warned risks of the Activity.
ASSUMPTION OF RISK
I agree to the Minor's participation at their own risk, understanding the inherent risks involved.
WAIVER, RELEASE & INDEMNITY
I release Hartley Golf Pty Ltd, its employees, and agents from liability for injury, loss, or damage, except where caused solely by their negligence.
I indemnify Hartley Golf Pty Ltd against all claims, except where injury, loss, or damage results solely from the negligence of the Business.
MISCELLANEOUS
The Minor agrees to comply with all rules and directions of the expert coach in charge.
I agree to report accidents, injuries, or loss to Game On Golf.
In case of injury or illness, I agree to cover the expenses for evacuation, first aid, and medical treatment.
I ACKNOWLEDGE THAT I HAVE READ THIS FORM. I FULLY UNDERSTAND ITS TERMS. I SIGN FREELY AND VOLUNTARILY WITHOUT INDUCEMENT.
