Workshop Participation Waiver
Welcome. On behalf of Licensed Willow EOL Educatorᵀᴹ - Jacqueline Williams ("Jacqui" trading as End of Life Transitions), we are honoured to be of service as you journey with end-of-life planning. We believe that consciously contemplating our inevitable death leaves us enriched and energized and more strongly connected to ourselves and others. Thank you for reading these Terms and Conditions. Why You’re Reading This Document: We want to make sure you’re aware of how Jacqui (End of Life Transitions) wants to assist and empower you and be clear on what Jacqui will not and cannot do for you. Jacqui (End of Life Transitions) facilitates opportunities for you to learn about yourself; to support you to get into action to holistically prepare for your inevitable death. Jacqui (End of Life Transitions) makes no representations that it offers any professional services in any area outside of the core competency of its business, chiefly advising and guiding people to make meaningful decisions about their life, end of life, and death, and does not represent or warrant that it has any expertise in the legal, accounting, financial planning, insurance, or healthcare fields. In offering its services, Jacqui (End of Life Transitions) is facilitating your learning and thought process about important decisions that you will make. We are not making any decisions for you and you acknowledge that you alone are fully responsible for any and all decisions that you make, no matter the outcome. Indemnity: You agree to indemnify Reena Lazar and Michelle Pante doing business as Willow End of Life Education and Planning and Jacqui (End of Life Transitions) and any of their employees, contractors, agents and assigns (the “Indemnified Parties”) from any and all claims brought against any of the Indemnified Parties arising from in-person and/ or online events or workshops provided by, Jacqui (End of Life Transitions). Release of Liability: You and any of your heirs, executors, administrators, successors and assigns forever release and discharge Jacqui (End of Life Transitions) from any and all actions, causes of actions, claims and demands which are based upon, arise out of or occur, directly or indirectly, by reason of any act or omission by you, unless Jacqui (End of Life Transitions) has contributed to such liability by an act of gross negligence. Intellectual Property Ownership: All logos, trademarks, photo and/or video assets, training and/or teaching materials, and all other intellectual property that Willow created is the sole and exclusive property of Willow End of Life Education and Planning and is for your personal use only. Media Release: You agree to grant Jacqui (End of Life Transitions) the irrevocable right and permission to use, throughout the world and in perpetuity, images, video content, motion pictures, photographs, videotapes, recordings or any other record of the Workshop, that we or any of our officers, directors, employees or agents have created, for any purpose, including commercial use. These rights and permissions include verbatim or paraphrased use of words you say aloud at the Workshop and write on any feedback and/or evaluation forms. Where appropriate, we will give anonymous reference to you as an attendee or participant of the workshop, unless we seek and get your permission to use your name. We will not use your name anywhere, without asking for your permission first. By registering for this workshop you agree to be bound by the provisions of this Agreement and for any future workshops you attend with Jacqui (End of Life Transitions)
