I, the undersigned, hereby acknowledge and agree that participation in soccer games, practices, and related activities carries inherent risks, including but not limited to physical injury. I understand that despite all reasonable precautions taken by the organizers, injuries may still occur.

In consideration of being allowed to participate in any way in the soccer programs, related events, and activities, I, the undersigned, acknowledge, appreciate, and agree that:

The risk of injury from the activities involved in soccer is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Peak performance 360, their officers, officials, coaches, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property associated with my presence or participation, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.


Participation and Safety:
All participants must adhere to the instructions and guidelines provided by the coaching staff.
Safety is our top priority. Players should wear appropriate sports attire, including shin guards, and follow safety protocols during training sessions and games.
Health and Hydration:
Players are encouraged to bring their own snacks and drinks to stay hydrated throughout the camp.
We recommend bringing a refillable water bottle to ensure proper hydration during activities.
Equipment:
Soccer players must bring their own soccer boots suitable for grass fields.
Shin guards are mandatory during training and game sessions.
Please label all personal belongings to avoid mix-ups.
Behavior and Respect:
Respectful behavior towards coaches, fellow players, and staff is expected at all times.
Bullying, foul language, or disruptive behavior will not be tolerated.
Players should actively participate, listen attentively, and follow instructions.
Attendance and Punctuality:
Players are expected to arrive on time for each session.
If a player cannot attend a session, please notify the coaching staff in advance.
Medical Conditions and Emergencies:
Parents/guardians must inform us of any relevant medical conditions or allergies.
In case of an emergency, we will follow established protocols and seek medical assistance if necessary.
Photography and Media:
By participating in the camp, players and parents consent to the use of photographs and videos for promotional purposes.
If you have concerns about media usage, please inform us in writing.
Payment and Refunds:
The camp fee is non-refundable unless the camp is canceled by the organizers.
Payment should be made in advance to secure a spot.
Lost Items and Liability:
The camp organizers are not responsible for lost or stolen items.
Participants and parents assume all risks associated with camp activities.

Our goal is to provide an enjoyable and educational soccer experience.
