You are attending and participating in Port Campbell Surf Life Saving Club Inc.'s (Club) Accessible Beach Day Guided Beach Walks at Port Campbell on Sunday, 3 December 2023 (Event). Please note:

The Event involves a 15 minute guided beach walk along Port Campbell Beach, from the Club to the suspension bridge and back.

PLEASE READ CAREFULLY

Please note that by participating in the Event and completing this Registration form, you:

a) accept the risks inherent in the Event;

b) waive and release, and will release SLSA, LSV and the Club and their respective personnel from any claims including but not limited to any negligent or reckless act or omission, the you may have, or may have had, but for this release arising from or in connection with your participation in any Life Saving activity conducted by the Club and/or LSV including the Event;

c) indemnify and keep indemnified SLSA, LSV and the Club and their respective personnel from any claims including but not limited to any negligent or reckless act or omission, that you may have, or may have had, but for this indemnity with your participation in any Life Saving Activity conducted by the Club and/or LSV including the event;

d) consent to the appointed doctors, coaches, staff and other officials of the Club to provide first aid to you and if required deliver you for treatment to the nearest operating and available medical facility if you are injured as a result of participation in any Life Saving activity conducted by the Club and/or LSV including the Event; and

e) if you have provided consent (via the question on the previous page of this registration form), you provide consent to having your name, image or likeness from your participation in this Event used to promote the Objects of the Club and/or LSV, by any form of media. You waive any rights you might have to or in such use of your name, image or likeness by the relevant lifesaving organisation.
