By booking for our Choir Season, you as a chorister and/or musician assume all risks related to the use of any and all spaces used by Perth Gospel Choir. I have been warned that this recreational activity may cause physical harm to me or to someone under my control or accompanying me, and I confirm that I have no condition, medical or physical, that could prevent me from, or be aggravated by, participating in this activity.
Examples of the risks I have been warned about include: tripping, falling and slipping; collision with someone or something; something giving way unexpectedly; back injury, strains; spillage and burns; trip hazards; electrocution or electric shock; car accident; traffic and parking incidents; accidents due to mobility aids; stage and rehearsal studio floors, surfaces and environments; bites and stings; sharp tools and equipment; sun exposure; poisoned by chemicals; injury from falling branches, equipment or more; strained back. I understand that Curate Arts Incorporated is insured and operates with a qualified conductor in a professional manner. In the event you observe unsafe conduct or conditions before, during or after any and all choir-related activities, you agree to report this unsafe conductor or conditions to the choir's conductor as soon as is possible.
