o Any class may be cancelled if the minimum number of enrolments is not received or facilitator is unavailable.


PUBLIC HOLIDAYS:
o There will be no classes on Public Holidays.


PRIVACY STATEMENT: The personal information requested on this form is being collected by Council for reference and identification purposes. To ensure confidentiality of information requested we will only use personal information provided by you for the purpose of providing a range of access to facilities and equipment. In accord with our Privacy Policy, we will not disclose your personal information without consent to a third party, institution or authority except where required by law or other regulation. You may apply for access and/or amendment to your personal information by contacting Council’s Freedom of Information Officer in writing.


PARENT / GUARDIAN CONSENT:
o I agree and consent to my child/ren participating in this program. I hereby also agree to release, discharge, indemnify and to hold the City of Brimbank and its servants and agents harmless for any accidents, harm, loss and damages which my above named child may suffer and/or sustain, which are in any way connected to, or as a result of, the said child participating in any way in this program, to the extent permitted by law.
o In addition, I hereby agree to indemnify the City of Brimbank and its servants and agents for any loss, demands, damages, expenses, claims actions and suits brought for and on behalf of the above child and arising out of or in any way connected to this program, to the extent permitted by law.
o I, the person with lawful authority to consent to the medical treatment of the child, hereby consent to centre staff seeking necessary emergency medical, hospital, dental, or ambulance services in the event of any form of illness or accident occurring involving the child, as the centre staff may determine is necessary, in their absolute discretion, at my sole cost and expense.


PHOTO CONSENT FORM - IMAGE/NAME USE:
Brimbank City Council needs your permission to use your and/or your child/ren’s name, image and digital recordings as part of Brimbank City Council media which includes (advertising, posters, brochures, websites, billboards and any other forms of media we may utilize from time to time). Information you provide as part of any interviews/filming may also be provided to third party providers or websites such as YouTube.

Photo Consent - Privacy statement:
By indicating your (and your child/ren's) Photo Consent preference during your TryBooking Registration, you (the legal guardian) consent to Brimbank City Council using the name, image and digital recording of the named party (you and/or your child/ren) for the purposes of promotion of Council activities and related purposes, which include but are not limited to distribution to third parties who may use the image for commercial purposes or where release of the image is required by law. You may withdraw your consent for future use by contacting the Council’s Privacy Officer or the Council department involved on 9249 4000 or directly contacting the West Sunshine Community Centre on 9249 4555 or wscc@brimbank.vic.gov.au.
During TryBooking Registrations, participants can provide:
o Details required of an adult being photographed - I am over 18 years
I consent to Brimbank City Council using my name and a photograph of me for the above uses.

o Details required for a person under the age of 18 years being photographed - I am the parent/guardian of a person under 18 years
I consent to Brimbank City Council using my child’s name and photograph for the above uses.
