By acknowledging these terms and conditions, you agree to the following:
1. I understand that these personal details with be added to the Jannali Anglican Church ‘It's Music Time’ database, email and SMS lists and that they will be used for the purposes of ‘It's Music Time’ and associated activities. I understand that I can unsubscribe at any time by contacting itsmusictime@jac.org.au.
2. I understand that I will be added to the email list for 'It's Music Time' at Jannali Anglican Church. I understand that I can unsubscribe at any time by contacting itsmusictime@jac.org.au.
3. I understand that photos and videos may be taken of my child during 'It's Music Time' and displayed publicly for the purposes of promoting 'It's Music Time.' I understand that I can withdraw my consent by contacting itsmusictime@jac.org.au.
4. I give permission for my child to attend and participate fully in all activities associated with ‘It’s Music Time’ at Jannali Anglican Church on the days in which they are registered.
5. I understand that an authorised, supervising adult must accompany my child and be responsible for his/her safety and participation whilst my child is attending ‘It’s Music Time’.
6. I assume full responsibility for my child's health being such that the activities of the program will in no way aggravate any condition present. If in any doubt, I will seek to follow medical advice and inform the leaders of ‘It’s Music Time’ of that advice. I will also notify the leaders of ‘It’s Music Time’ of any significant change in my child’s health prior to the commencement of the program on the days in which they are registered.
7. I agree to abide by the COVID-safe measures in place at the Jannali Anglican Church.
8. In the case of a medical emergency, I give permission for the leaders of 'It’s Music Time’ to seek medical aid for my child (including, but not limited to hospitalisation and surgery), and while I understand that every effort shall be made to contact me prior to such procedures, I agree to pay all such doctors’ and hospital fees incurred on behalf of my child.
9. I understand that information may be shared with a third party for the purposes of providing adequate medical care.
10. I understand that while all precaution shall be taken to ensure the good welfare and protection of my child, the Anglican Diocese of Sydney, its council members, staff members, employees, or any other person acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to my child or damage or loss to their property.
11. I agree to indemnify and hold harmless Jannali Anglican Church against all claims, demands, suits and liability of injury to my child or myself whilst attending ‘It’s Music Time’.
