Refunds are available at the discretion of Campbell’s Athletic Academy or please choose carefully as there are no refunds. This ticket may be valid as a tax invoice or receipt. This ticket has been created and managed using the TryBooking site and is also subject to the Terms and Conditions of TryBooking. As the Event Organiser hosting this sports program, Campbell's Athletic Academy is solely responsible for all issues relating to the sale of tickets. By purchasing this ticket you are giving your child permission to participate in this sports program. Campbell’s Athletic Academy does not have personal accident insurance cover for students. If the student is injured as a result of an accident or incident while participating in the sports program, all costs associated with the injury, including medical costs are the full responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may also be covered by your provider. Any other costs must be covered by parents/carers. Please take this into consideration in deciding whether or not to allow your student to participate in this sports program. Campbell’s Athletic Academy (including but not limited to, any photographer, interviewer, creative agency or media organisation) has all rights to recording images of the student for promotional purposes. These images will be used by Campbell’s Athletic Academy and disclosed to any person or organisation approved by Campbell’s Athletic Academy, including but not limited to, publishing them as part of a book, poster, brochure or report, newspaper advertisement or article, television advertisement or program, radio advertisement and any other media. Campbell’s Athletic Academy and their agents may edit the images prior to publication, as they consider appropriate, without first consulting me. In the event of an accident or illness, Campbell’s Athletic Academy staff may obtain or administer any medical assistance or treatment the student may reasonably require, including contacting an ambulance all costs associated with the injury, including medical costs are the full responsibility of the parent/carer. Any data submitted on this form is collected by Campbell’s Athletic Academy for consent to the use, disclosure and publication of the student. The information will not be disclosed to other parties except where permitted under the Privacy Act 1988.
