I am aware that the details may be shared with Netball Victoria and any other affiliated netball association. Two Bays Region Officials and Team Officials will also have access to this information and any additional medical information provided

In the event of illness or injury to my child whilst at Netball or travelling to or from Netball; I hereby authorise a Two Bays Region Official or Team Official, where it is impracticable to communicate with me/or a parent, to obtain such medical attention and/or hospitalisation as are deemed necessary and agree to pay all expenses incurred

I am aware that Netball Victoria membership/insurance must be paid before taking part in any training and/or matches and accept all responsibility for the consequences of it not being paid

A condition of participation in the Two Bays Team includes adhering to and informing spectators you may bring to netball codes of conduct, bylaws and policies.

I agree that images and/or names of Two Bays Region participants, including myself/my child, may be used before, during or after the State Titles for promotional, broadcasting or reporting purposes in any media, including the Two Bays website and social media pages

I agree that if selected fees and costs associated will be paid in full.
