DEED OF ACKNOWLEDGEMENT
As a condition of attending and participating in the FREE INFO SESSION ("Info Session") facilitated by Rhonda Muller ("Facilitator"), I hereby acknowledge and agree:
Restrictions:
I will not photograph, film, distribute, modify, communicate, reproduce, publish, or otherwise deal with the Info Session material or knowledge without prior knowledge and the express written permission/consent of Rhonda Muller.
Participation
I will act reasonably in accordance with any instructions given to me during the Info Session to allow the full benefits of the Info Session to be achieved. I will not hold any person involved with providing the Info Session liable for any injury, harm, loss, consequential loss or expense as a result of participation in the Info Session or use of the Facilitator's material and knowledge, except where such liability cannot be excluded by law.
Media Permission
SMILE! Your attendance at events by Rhonda Muller and/or ASD Healthy Life may be digitally recorded through photographs and/or video recordings. Images may be used on Rhonda Muller's and/or ASD Healthy Life's websites, publications, social media or by newspapers. By attending this session you agree to these terms. If you do not wish your image to be published, please notify staff before the event.
1. In the promotion of the Info Session we may want to use photographs / video footage of your participation and debrief of the Info Session.
2. The document gives Rhonda Muller permission to use still or moving pictures of you for purposes associated with the promotion of any Info Sessions facilitated by Rhonda Muller and/or ASD Healthy Life - Info Sessions / Workshops.
3. By purchasing this ticket or securing your place at this event, you are agreeing that Rhonda Muller and/or ASD Healthy Life has permission to use your image and/or video footage for the purpose mentioned. Rhonda Muller may record any sound / or vision of you whilst you are completing the Info Session activities.
4. You give permission to Rhonda Muller to use this sound / vision for:
a. Media activities such as newspaper articles, brochures, television advertisements and promotional material;
b. Future media activities which would assist with further promotions for Rhonda Muller;
c. Promoting and advertising of the Info Session.
5. You understand that you will not be paid by Rhonda Muller for giving this permission.
6. No photo's and/or filming to be taken without prior knowledge and written consent of any event Rhonda Muller facilitates.
Please note: Non Attendance/Cancellation Policy
Notification of cancellation received in writing no less than 3 days prior to the Info Session/Workshop will be refunded less 25% processing fee (where a fee is required). No refunds will be given within 72 hours of the workshop.
