Swim Skills clinic Participation Consent
I acknowledge that by participating in the Skills Clinic and related activities as organised by Rackley Swimming, that in addition to the usual risks inherent, certain additional risks and dangers may be encountered which may include: physical exertion, weather extremes due to sudden and unexpected change; evacuation difficulties if the participant is injured.
I understand that personal injury can and may occur to my child, and I hereby authorise Rackley Swimming and it’s appointed Clinic Coaches, to seek and consent to emergency medical attention for my swimmer as needed (including ambulance and hospitalisation); and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.
I will notify the Swimmers Coach if I/my child has had contact with any infectious disease within (4) weeks of the Clinic.
I have provided above all of the relevant information, restrictions, allergy and medication information necessary for my child to receive appropriate medical care, if it is needed.
