Thank you for registering to attend a Centenary Archers Club archery class.

COURSE LOCATION: Lower Field, 427 Wacol Station Road, Sumner Q 4074
COURSE TIME: Arrive *15 minutes* _before_ the start time. The course is *3+* hours or more. _Arrivals 15 minutes after the start time will not be permitted to join the class. NO REFUND for late arrivals or 'no shows'._

You MUST be aged 10 or older, unless with prior agreement
You MUST NOT be under the influence of alcohol or drugs.
You MUST wear fully enclosed footwear.
Tie back long hair & remove necklaces, ear, nose and lip rings so they do not get caught in the bow string.
* Bring sunscreen & a hat. _(soft school hats are best, baseball caps are not suitable as they provide little sun protection and interfere with the bow string)_
* Guardians of participants 16 years and under MUST remain on site during the class.

I have read the above instructions & requirements and will follow & accept them. I confirm the participant is 10 years old & over, or If you are younger than 10, you have consulted with the club beforehand and obtained an okay from us.

I accept that the club may take & display images taken of participants for the purpose of promoting the sport, club, events or activities on the web or in house or external publications and magazines.

I accept that physical contact may be necessary when correcting posture & fitting equipment. (Please let our instructors know in advance if this is an issue.)
