As the parent and/or legal guardian of the applicant, I give my consent for them to attend and participate in all such activities associated with IGNITE 2026 on the days for which they are registered. I assume full responsibility for the applicant's health being such that the activities of the program will in no way aggravate any condition present. If in any doubt, I will seek to follow medical advice and inform IGNITE 2026 of that advice. I will also notify IGNITE 2026 of any significant change in the applicant's health prior to the commencement of the program. I declare all information provided on the applicant's health to be true and accurate and that all relevant information has been provided.

I understand that personal accident insurance is the responsibility of the individual applicant, and that while all precaution shall be taken to ensure the good welfare and protection of the applicant, the Anglican Diocese of Sydney, its council members, staff members, employees, or any other person acting on their behalf are hereby released from any and all liability in the event of any accident or misfortune that may occur to the applicant or damage or loss to their property.

In case of an emergency I give permission for the IGNITE 2026 directors to seek medical aid for the applicant (including, but not limited to hospitalisation and surgery) and while I understand that every effort shall be made to contact me prior to such procedures, I agree to pay all such doctors' and hospital fees incurred on behalf of the applicant.

I understand that photos and video footage of activities may be taken and viewed within the youth ministry groups of St.Clement's Anglican Church Jannali and/or for future promotional purposes. I further understand that no such visual imagery recorded by IGNITE 2026 leaders which singles out the applicant will be uploaded to the internet.

I understand that youth will be transported to some activities in leaders' cars and that no youth will be driven by Red P drivers.
