I hereby consent and agree that my Club, Netball WA and Netball Australia have the right to use my name, personal and contact details to inform and liaise with me in relation to all club activities.
I hereby consent and agree that my Club, Netball WA and Netball Australia have the right to take or use photographs and video of me and to use these in any and all media publications.
I acknowledge that I am exposed to certain risks during Netball and other Club Activities and that I (on behalf of myself, my heirs, executors and administrators) hereby waive any and all Claims which I or they might otherwise have arising out of loss of life, injury, damage or loss or any description whatsoever which I may suffer, sustain, or incur in the course of participating in activities of the Club.
I acknowledge that the Club is not liable to me or any other person for damage or theft to, or loss of, any property at activities of the Club, regardless of the cause.
I agree to follow all Club policies and procedures.
