PRIVACY DECLARATION
Willoughby Park Anglican Church (WPAC) seeks to deal carefully with the personal information we collect and to do so in a manner consistent with reasonable expectations. The personal information in this form will be made available to:
(a) the WPAC leaders involved in the running of the activities in which my child participates, and
(b) medical and emergency services personnel if considered necessary.
PHOTO CONSENT
I give permission for photos and videos of my child taken at WPAC events to be displayed publicly (online and in print) unless I advise the WPAC leaders otherwise.
AUTHORISATION & EXPECTATIONS
• I give permission for my child to attend the Kids' Christmas Workshop at WPAC on Monday 22 December 2025.
• I authorise the WPAC leaders, in the event of an emergency and where they are not able to communicate with me immediately, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.
• I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in my child being returned to my care. (NB This provision will only be applied in the most extreme circumstance as determined by the program director.)
• I will provide the WPAC leaders with any information relevant to the well-being of my child prior to him or her attending the Kids' Christmas Workshop.
• I confirm that the information given in this form is true and correct, and will advise WPAC of any changes to this information.
