Medical Condition and Age Declaration
Archery involves a level of physical activity such as drawing and holding the bow and a reasonable amount of walking. It also requires a normal level of sight and hearing. You should also bear in mind the possibility of injury.

If you have a medical condition which could give rise to problems, you must provide a certificate from you doctor before you commence.

I declare that as a class attendee I am at least 7 years old. If the attendee is under 18, a parent or legal guardian confirms and agrees to this declaration on their behalf.

I agree to the above statement regarding age and physical condition.

Please note that “REFUNDS/RESCHEDULES: refunds are only available if non-attendance is advised at least 24 hours prior to the session.
