- I give permission for my child to attend Summer Fest 2025 with Grace Youth from Friday 24th Jan to Sunday 26th Jan. I understand it is my responsibility to organize transport to Covenant College on Friday 24th 7pm, Sat 25th 10am and Sun 26th 10:30am and from Covenant College Friday 24th and 25th 9pm, and Sunday 26th from Adventure Park Geelong 5pm

- I understand that this is a Christian event and will involve the engagement with a Christian worship environment. I understand the Church will take reasonable steps to provide a safe environment for my child, and to ensure that all equipment supplied by them, for activities, is of a reasonable standard. I understand the Church will not be liable to any injury that may be suffered by my child, which arises either directly or indirectly from, or in connection with, the activities organized and run by the supervisors of the Youth Ministry or Conference Organizers. *

- I agree that the delegated leaders may authorize on my child’s behalf any medical treatment that he/she may require, including calling an ambulance in the event that I can not be contacted and the leaders of the activity deem the situation to warrant the call.

- I understand that I may have to come and see to the medical needs of my child if the leader of the group deems it necessary

- I give permission for my child to be transported to the location of any event in a bus or car and I release Grace Church from any liability resulting from injury to or from the activity location
