Indemnity
By signing this document, I agree to indemnify the City of Brimbank and its servants and agents against any accidents, harm, loss and damages which I may suffer and/or sustain, which are in any way connected to, or as a result of, the said participating in any way in this program, to the extent permitted by law.

In addition, I agree to indemnify the City of Brimbank and its servants and agents for any loss, demands, damages, expenses, claims actions and suits brought for and on behalf of the above and arising out of or in any way connected to this program, to the extent permitted by law.

I, the person with lawful authority to consent to the medical treatment and consent to centre staff seeking necessary emergency medical, hospital, dental, or ambulance services in the event of any form of illness or accident occurring, as the centre staff may determine is necessary, in its absolute discretion, at my sole cost and expense.

Payment
Cash payments to be made in person. EFTPOS and Credit Card payments may also be made online through the relevant program TryBooking link.
In person payments to be made at Westvale Community Centre (between 9.00am – 5.00pm Monday to Friday).
Agencies wishing to book clients are advised to email (westvale@brimbank.vic.gov.au) participant enrolment form and agency details and an invoice will be remitted.

Class Cancellation
• Enrolments are assessed by the number of fees paid.
• Any program may be cancelled 3 working days before the start date if the minimum number of paid enrolments has not been received.
• Participants enrolled in a cancelled course will be notified and fully refunded.

Refund
If a participant withdraws from a course, refund is payable in special circumstances only, and by written application. Once a course has begun, no refund is possible. A $10.00 administration fee is deducted on all refunds unless Brimbank City Council is liable for cancellation.
Public Holiday: No programs are run on Public Holidays

Customer Behaviour
One of Council’s goals is to create an inclusive community by supporting access, equity, community participation and human rights for all citizens. However, there may be situations where Council chooses to ban a customer displaying inappropriate behaviour from using specific Council services for a specific period of time.

The personal information requested on this form is being collected by Council for reference and identification purposes. To ensure confidentiality of information requested we will only use personal information provided by you for the purpose of providing a range of access to facilities and equipment. In accord with our Privacy Policy, we will not disclose your personal information without consent to a third party, institution, or authority except where required by law or other regulation. You may apply for access and/or amendment to your personal information by contacting Council’s Freedom of Information Officer in writing


TryBooking is an online system for organisations and community groups that has been designed to look after the entire event ticketing and registration needs.

All issues relating to the sale of tickets and the hosting of their associated event(s) are the responsibility of the “Event Organiser” and not that of TryBooking Pty Ltd (Australian Business Number 71126987915).
