Disclaimer:

1. I, the above signed in consideration of, and a condition of acceptance of my registration in the above event for myself, my heirs, executors and administrators, hereby waive all and any claims, fight of cause or action, which I or they might otherwise have arisen out of any loss of life or injury, damage or loss of any description whatsoever which I may suffer or sustain in the course of or consequent upon my attendance in the said event. I, the undersigned am fit and healthy to partake in the event and I have no serious medical condition.

2. This waiver, release and discharge shall be and operates separately in favour of all persons, corporations, and bodies involved or otherwise engaged in promoting or staging the event, including but not limited to the Australasian Federation of Traditional Karate and Kobudo, its presenters, host venue, and Management Committee.

3. I declare that all the information given on this form is correct.

4. I declare that I am medically fit and able to participate in any/all training activities, and any adjustments made to training will be on my own management, and that I shall communicate any/all injuries that may affect my training to the presenters and/or training partners, where necessary or when modification to activities are required.

5. I declare that I understand, agree and will abide by the AFTK's Policy Statement, as found at https://aftk.org.au/about/

Ticketing, Attendance and Administration:

1. Refunds cannot be made on cancellation of registration. If the event is cancelled by the AFTK, refunds will be provided to all participants.

2. Personal information supplied on this form will only be used for planning the event, and sharing of future AFTK event details of the same nature to which you registered for.

3. The AFTK is not liable for any costs associated with attending the seminar, and is not liable for any expenses should the in-person seminar be cancelled due to COVID-19 or Public Health Directions. The AFTK will make all effort to ensure the seminar is accessible online should it not be presented in-person.
