The instructors and all associates at Maryborough Martial Arts shall not be liable for any personal injury or death and/or loss of property, suffered by the student or instructor, or any other student.
I certify that I am / my child is physically able to participate and do hereby agree that Maryborough Martial Arts and its employees are not responsible or liable to me for any injury, accident, death or loss of personal property. I do hereby absolutely release and discharge Maryborough Martial Arts, its owners and employees from all liability whatsoever arising in respect of my enrolment with Maryborough Martial Arts and knowingly participate at my own risk as a student of Martial Arts.
I give my consent for Maryborough Martial Arts to use any photographs and/or video in which I or my child appear for the use of Publications and Marketing.
