Glen Eira City Council (Council) is collecting your personal and health information through Trybooking for the purpose of administering and managing your participation in the Council staff reformer pilates and yoga program (the Program). This information includes your name, contact details and confirmation you are physically fit to participate in the Program. Additionally, during the Program, or in connection with the Program, you may choose to disclose further health information to Council.

Any personal or health information provided in connection with the Program will only be used for the intended purpose and will be handled in accordance with the Privacy and Data Protection Act 2014 (Vic) and the Health Records Act 2001 (Vic).Your personal and health information may be shared with relevant Council staff, including the program facilitators, as necessary for the Program delivery but will not be disclosed to any other third parties without your consent unless the use or disclosure is required by law or otherwise permitted by the Act.

If the personal and health information in this form is not collected, we may not be able to process your registration or include you in the Program.
